Welcome to Lighthouse Online Return Material Authorization Request Form.
The following step by step process will provide you with a Return Material
Authorization Number. This RMA Number can be used to reference the progress
of repair and calibration of your unit. Please reference this RMA Number
when contacting us. We cannot accept material being returned without a RMA
Number.
In order to complete the form, you will need the following:
1. The Model Name of the item you want to return.
2. The Serial Number of the item you want to return.
3. A purchase order number or credit card information, unless the item you want to return is under warranty. Determination of warranty coverage will be made when the item is inspected at a Lighthouse Worldwide Solutions Facility.
4. Important: Please click and download the Safe Material Handling Form. Customer will need to print, sign and return the form with the instrument.
